- We are currently accepting new patients.
- Anybody can join the practice if you live within the practice area. For our practice area, please click here.
- If you want to join the practice please read this guide to joining as a new patient. You can join by completing two forms:
- a registration form which can be downloaded here or if this is for someone under the age of 5 please click here AND
- you will need to complete a GMS1 form here.
You will need one copy of each form per person. These are also available in Reception.
- If you are on regular tablets or medication when you join the practice then our practice pharmacist will contact you on your preferred telephone number to discuss this with you. You do not need to make an appointment just for this.

Please bring the completed form along to reception. Or you can email the completed forms to nnccg.cvamp@nhs.net. You do not need to bring any ID or proof of address or immigration status. If you have a medical card please bring it with you as the information on the card will speed up the process of getting your records from your previous practice. Once you have joined, your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be polite to do so.
